Everything you need is on one mobile-friendly form. Information stays private and is used only to confirm eligibility and coordinate the program.

Program information

Please read these policies before completing the registration form.

Program eligibility and service area

Children must be under 18 and live within our local service area. The service area runs from Deseronto north through Stone Mills, east through Verona, Battersea, and Joyceville, south through Kingston, and west to Bath. If you live near the boundary, contact us before applying. Children must not be enrolled in another holiday assistance or gift program during the same season.

Identity and address verification

Guardians must provide photo identification and proof of address. A driver’s licence or Ontario Photo Card meets both requirements. A passport must be paired with a recent phone, utility, bank, or government document showing the guardian’s name and home address. If photo identification is unavailable, provide two recent documents showing the guardian’s name and address. Each child requires a birth certificate or passport showing their full name and birthdate. Contact us before applying if these documents are unavailable.

Health cards are not accepted

Do not upload or send an Ontario health card. Storms Miracle Tree does not accept health cards or health card numbers for identity verification. Contact us to discuss another document.

Double-dipping policy

To help gifts reach as many families as possible, please do not register if you are receiving holiday assistance through another program. Storms Miracle Tree works with other organizations to track duplicate registrations. Registering with multiple programs may result in removal from this program and others, and the household may be flagged for future seasons.

How the waitlist works

Some children may be placed on a waitlist until sponsorships become available. The team will do its best to fulfill as many wishlists as possible, but placement on the waitlist does not guarantee sponsorship.

Privacy and verification

The legal names of both the child and guardian are required for verification only and remain confidential. Sponsors see an anonymous child ID, age, gender, and wishlist—not names, addresses, schools, or family contact information.

3 Secure verification documents

Your documents stay private.

Only signed-in administrators access these files. The system deletes them after verification, or after 30 days if review has not been completed. Files never appear in email or on the public website.

Do not upload a health card. Health cards and health card numbers are not accepted.
